Diagnostische Charakteristika früher Arthritiden und Spondarthritiden

Abstract
Arthritides and spondylarthritides in their early stages are a problem frequently facing the rheumatologist. New diagnostic possibilities have improved the identification of spondylarthritides and reactive arthritides. 320 patients whose disease had lasted for less than a year were examined during a consulting-room schedule for eary diagnosis of arthritis. 33% of these patients were definitely identified as suffering from an inflammatory rheumatic disease that could be classified, whereas such classification was not possible in 37%. This means that 70% of all diagnoses concerned inflammatory diseases. 13% had degenerative diseases, osteoarthritis and tendomyopathies, whereas 17% had other diagnoses. Rheumatoid arthritis was confirmed in 18% and considered possible in 7%, reactive arthritis was definitely diagnosed in 11% and possible in 11%. Undifferentiated arthritides and spondylarthritides were most frequently represented among the diagnoses (25%). If these are included, we arrive at a figure of 52% of inflammatory rheumatic diseases of less than a year''s duration that could not be definitely classified. The characteristics of the undifferentiated arthritides are presented. The validity of the revised criteria of the American Rheumatism Association (ARA) was checked. The sensitivity was 94% and the specificity 90%. Using the criteria for reactive arthritis of the French Society for Rheumatology the sensitivity was 92% and the specificity 99%.This publication has 0 references indexed in Scilit:
