Abstract
This chapter considers evidence suggesting that there are important common threads that underlie the pattern of inequalities in mortality within and between countries. Some of these common threads involve the action of factors that span the life course of individuals. The assumption that health inequalities, whether within or between countries, are caused by contemporaneous differences in circumstances is not sustainable for a range of important diseases that appear to be driven instead by poor socio-economic circumstances in early life and adulthood. Keywords
