Abstract
Lung scans at 24 hours after the onset of infusion indicated that restoration of pulmonary arterial blood flow was more rapid in patients who received a 12-hour urokinase infusion than in patients treated with heparin alone. Patients with recent emboli (less than 48 hours old) appeared to respond better to urokinase than patients with older emboli (48 hours to 6 days old). After 7 days, there was no difference in lung scan resolution between the urokinase and the heparin patients, and both groups showed progressive lung scan improvement for approximately 3 months. Regardless of treatment, prominent residual scan defects (≥10%) remained in 16% of the patients at the end of a year. Six of the 17 patients with such defects had a history of pulmonary embolism preceding the event that brought them into this Trial.
