Laparoscopic Pyloromyotomy: The Slice and Pull Technique

Abstract
From August 1995 to April 1996 20 patients underwent laparoscopic pyloromyotomy. Ages ranged from 3 weeks to 3 months and weights from 2.1 to 5.4 kg. The technique involved the use of one 3.5-mm trocar, a single 3.4-mm babcock clamp, a 2.7-mm or a 1.6-mm zero degree telescope with accompanying sheath, and a 3-mm serrated sheathed arthroscopy knife. AH procedures were completed successfully. The average operating time was 13 min. Feeds were started 4 to 8 h postoperatively and advanced on a 2 h basis. Nineteen of the twenty patients were discharged in under 24 h and one was kept until postoperative day 2 because of feed intolerance. There were no complications. This technique for laparoscopic pyloromyotomy is fast, safe, and effective, and can be performed with standard laparoscopic tools.
