Abstract
THE infrequency with which a firm clinical diagnosis of pyelonephritis can be made without histologic examination of renal tissue prompted the present study. The urinary sediment might be expected to be of value in determining renal disease and the so-called pus or polymorphonuclear-leukocyte cast has been considered characteristic of active pyelonephritis. However, even with the most careful scrutiny these casts frequently cannot be identified in pyelonephritic urines by ordinary methods. Furthermore, the differentiation of the polymorphonuclear cast from one composed of renal tubular epithelial cells is often impossible.
