Abstract
• A patient with vigorous achalasia demonstrated manometric response to administration of atropine sulfate and nitroglycerin. Clinical improvement was achieved with the use of an oral anticholinergic agent and long-acting nitrates. The relationship of vigorous achalasia and diffuse esophageal spasm is discussed.
