Abstract
The 1970 Seven Crowns Reform in Sweden instituted a uniform low outpatient fee for patients, abolished fee-for-service outpatient work, put all government-employed physicians on full salary, and equalized salaries among different specialties. Five basic problems lay behind the formulation of the Reform: rising costs; high physician incomes; physician elitism; inequality in ambulatory clinics; and cumbersome insurance.
