Epilepsy with Multiple Sclerosis

Abstract
SUMMARY: Ten patients with multiple sclerosis and convulsive disorders are presented. In half the patients despite generalized convulsions the EEG revealed focal abnormalities which were variable during the evolution of the disease. Epileptic manifestations were often the first symptom of the illness; less often they accompanied exacerbations.There were two groups, each with its own prognosis. In the first group the seizures were rare and readily suppressed by treatment. In the second group the seizures were refractory to treatment, were associated with pronounced mental disorders or clouding of consciousness and with meningo‐encephalitic episodes to comprise a syndrome similar to Schilder's disease; neuropathological examination of the brain of one of these patients demonstrated massive centro‐lobular destruction by plaques.RÉSUMÉ: A propos de 10 observations de sclérose en plaques, comportant des manifestations épileptiques, les auteurs montrent, que dans la moitié des cas, malgré le caractère clinique apparemment généralisé des crises, l'E.E.G. décèle des anomalies focales, variables d'ailleurs au cours de l'évolution.Les manifestations épileptiques sont souvent le premier symptôme de la maladie; elles accompagnent plus rarement les poussées de l'affection.Deux types s'opposent nettement sur le plan du pronostic. Dans un premier groupe, les crises sont rares et disparaissent avec le traitement; dans un deuxième, tout aussi important, les crises sont rebelles à la thérapeutique, s'associent à des troubles psychiques importants, et à des épisodes méningo‐encéphalitiques, réalisant un syndrome voisin de la maladie de Schilder, comme semble le confirmer l'étude neuro‐pathologique d'un cerveau, montrant une atteinte centrolobaire particulièrement massive.Keywords
