Abstract
Presentation of CaseA 76-year-old woman was admitted to the hospital because of visual loss. She was well until 30 years previously, when she experienced the sudden onset of intermittent diplopia. Spectacles with prisms were prescribed, with improvement. Five years later she began to have right frontal headaches, which were often present on awakening and were relieved by aspirin. Twenty years before admission the diplopia became more prominent. She came to this hospital, where examination revealed partial paralysis of the left oculomotor nerve and questionable paralysis of the right oculomotor nerve. A neostigmine test was positive.
