Erste klinische Erfahrungen bei System-Mykosen mit einem neuen oralen Antimykotikum

Abstract
Eine 59jährige Patientin mit sekundärem Aspergillom, eine 62jährige Kranke mit einer Candida-Infektion der Bronchien und ein 37jähriger Patient mit einer Sycosis barbae candidamycetica wurden mit einem neuen Antimykotikum (BAY b 5097) behandelt. Die Substanz erwies sich in diesen Fällen als sehr wirksam. Die Verträglichkeit, beurteilt an Leber- und Nierenfunktion sowie Blutbild, war auch nach langfristiger Anwendung zufriedenstellend. Intestinale Störungen können auftreten. Zum Therapieerfolg sind relativ hohe Dosen erforderlich; sie wurden in den besprochenen Fällen aus der Relation der minimalen Hemmkonzentration der Pilze in vitro zum Serumspiegel errechnet. Der fungistatische Wirkungstyp von BAY b 5097 kann bei zu kurzer Therapiedauer zu Rezidiven führen. A new antimycotic preparation (BAY b 5097) was successfully used in the treatment of secondary aspergilloma in a 50-year-old woman, of candida infection of the bronchi in a 62-year-old woman, and of Sycosis barbae candidamycetica in a 37-year-old man. Judging by liver and renal function tests and the blood picture, the drug was well tolerated even after long-term administration. Intestinal side-effects may occur. Relativey high doses are required for successful treatment. In the described cases dosage was calculated from the relationship of the mean inhibitory concentration in vitro to the serum level. Short duration of treatment may lead to recurrence. Primeras experiencias clínicas en las micosis sistematizadas con un nuevo antimicótico oral Una paciente de 59 años con un aspergiloma secundario, otra de 62 años con una infección bronquial por cándida y un paciente de 37 años con una sicosis candidamicética de la barba fueron tratados con un nuevo antimicótico (BAY b 5097). La substancia se mostró muy eficaz en estos casos. Incluso la aplicación a largo plazo era satisfactoria a juzgar por el estado de las funciones hepática y renal así como por el cuadro hemático. Pueden presentarse trastornos intestinales. Para el éxito terapéutico son necesarias dosis relativamente altas; las dosis fueron calculadas en los casos descritos partiendo de la relación de la concentración hemática mínima de los hongos in vitro con el índice sérico. El modo fundistático de actuar del BAY b 5097 puede conducir a recidivas, si el tratamiento es demasiado breve.
