Nachweis von humaner Papilloma-Virus(HPV)-DNA in Vulvakarzinomen

Abstract
18 invasive maligne Tumoren der Vulva und 2 vulväre intraepitheliale Neoplasien wurden mittels Southern-Blot-Hybridisierung auf das Vorkommen von humaner Papillomvirus (HPV)-DNA untersucht. In 6 von 11 primären Plattenepithelkarzinomen, darunter einem Karzinom der Bartholinischen Drüse, ließ sich HPV-16 nachweisen. Hingegen enthielten alle 7 untersuchten Rezidivtumoren, darunter 5 Plattenepithelkarzinome, 1 Fibrosarkom und 1 Zylindrom keine HPV-DNA. Das Durchschnittsalter der HPV-positiven und der HPV-negativen Patientinnen unterschied sich nicht signifikant. Es fand sich keine Korrelation des HPV-Nachweises mit dem Differenzierungs- und Verhornungsgrad der Tumoren, bzw. mit dem Tumorstadium und dem Lymphknotenstatus. In einem tumorbefallenen Lymphknoten ließ sich ebenso wie im zugehörigen Primärtumor HPV nachweisen. 12 Proben von tumorfreier Abdominalhaut als korrespondierendem Normalgewebe enthielten keine HPV-DNA. Eine Korrelation des klinischen Verlaufes mit dem HPV-Status des Primärtumors ließ sich nicht nachweisen. Using Southern Blot hybridization 18 invasive malignant tumours of the vulva and 2 intraepithelial vulvar neoplasias were tested for the prevalence of HPV-DNA. In 6 of 11 primary squamous carcinomas, including one carcinoma of the Bartholinic gland, HPV 16 could be identified. Contrarily, in all seven recurrent vulvar tumours examined, including 5 squamous carcinomas, 1 fibrosarcoma and 1 zylindroma, no HPV-DNA was found. No correlation existed between HPV status of the tumours and grade of differentiation or keratinization, or the tumour-stage and lymphnode status. In one tumour-invaded lymphnode HPV 16 was detected as in the corresponding primary tumour. 12 samples from tumour free abdominal skin considered as corresponding normal tissue specimens were negative for HPV-DNA.
