Die traumatischen Rupturen des Isthmus der Aorta anhand von 21 operierten Fällen

Abstract
Von 1965 bis September 1976 wurden 21 traumatische Rupturen des Isthmus der Aorta operiert, davon 13 in den Jahren 1975 und 1976. Es handelte sich 15mal um eine frische Ruptur und 6mal um ein chronisches falsches Aneurysma. Eine Angiographie wurde in jedem Fall durchgeführt. Die assoziierten Schäden waren häufig, und 6 Patienten hatten eine Laparotomie vor dem Eingriff an der Aorta. Dieser wurde in 20 Fällen unter teilweiser extrakorporaler Zirkulation durchgeführt und bestand in 12 direkten Nähten der Aorta (11 frische Rupturen und 1 chronisches Aneurysma) und 9 Interponierungen von Prothesen (5 chronische A. und 4 frische Rupturen). Es gab 4 Todesfälle und 17 Überlebende, davon 16 ohne Komplikation. Ein Patient leidet an einer residuellen Paraplegie. From 1965 to December 1976, 21 traumatic ruptures of the descending thoracic aorta have been operated; the last 13 cases have been managed in 1975 and 1976. The lesions consisted in 15 recent ruptures (R.R.) and 6 chronic aneurysma (CA.). Associated injuries were the rule and 6 patients underwent exploratory laparotomy prior to thoracotomy. Surgical repair was consistently due by use of an extracorporeal by-pass (20 times), and resulted in 12 end-to-end aortic sutures (11 R.R., 1 C.A.) and in 9 reconstruction by means of a tubular dacron graft (5 CA., and 4 R.R.). 4 patients died. 16 of the 17 survivals are healthy and active. Paraplegia developed in one patient.
