Abstract
Presentation of Case First admission. A sixty-seven-year-old widow was referred to the hospital because of dyspnea.One week before entry the patient began to be dyspneic upon climbing one flight of stairs, with an associated sensation of lightheadedness and smothering. Despite restricted activities she continued to experience mild episodes of dyspnea once or twice daily, and paroxysmal nocturnal dyspnea developed. Two days before admission sharp pain in the left inframammary area on deep inspiration and a hacking, nonproductive cough appeared. On the night before entry she was unable to lie down because of marked dyspnea and a sensation of pounding . . .
