A Community-Oriented Geriatric Rehabilitation Unit in a Nursing Home

Abstract
Described is the first 16 months of operation of a nursing home-based community-oriented rehabilitation unit that utilized an interdisciplinary team approach in its admission evaluation and in its patient management decision-making. Of the 47 patients admitted in a 16 month period, 27 (57%) were discharged home after an average stay of three months in the rehabilitation unit. Of the 27 patients discharged to home 23 (85%) were still in their homes after an average post-discharge period of eight months whereas only 4 required readmission to other chronic care facilities.
