To the Editor: The case discussion by Dr. Dawson regarding the gentleman with Fabry's disease was very enlightening (Jan. 12 issue).* I would like to clarify a point about the patient's gaze palsy. The explanation reads, "The limitation of rightward gaze implies either a lesion of the left cerebral hemisphere, particularly in the frontal or the occipital region, or a disorder of the right pontine tegmentum." One must first know whether this is a limitation of smooth-pursuit or saccadic eye movement. Asking the patient to follow a finger to the right challenges the smooth-pursuit system controlled by the right pontine . . .
