Transcatheter arterial chemoembolization (TACE) in the treatment of unresectable liver cancer

Abstract
Basis, techniques, and recent strategies and results of interventional treatments for hepatoma were reviewed. The basic experimental researches indispensable to justify clinical technique and dosage have been accomplished. A total of 1310 unresectable cases underwent transcatheter arterial chemoembolization (TACE) and/or related therapies. The related therapies are several modified or strengthened TACE such as Lipiodol-TACE and subsegmental TACE. The therapies for unembolizable cases are balloon occluded arterial infusion, Lipiodol-SMANCS infusion and percutaneous direct injection chemotherapy, that reinforces the effect of TACE. As for the resectable cases, postoperative TACE was proved to be beneficial because it increased survival rates; however, preoperative TACE was not beneficial. The TACE and related interventional radiologic therapies for the hepatoma can be an equal or superior therapy by virtue of targeting chemotherapy. Les concepts des techniques de base et les résultats des différentes stratégies de traitement du carcinome hépatocellulaire (CHC), la recherche fondamentale, indispensable pour déterminer la technique clinique et les dosages sont revus. Nous avons traité 1310 cas de CHC par chimioembolisation artérielle (CEA) et/ou une thérapeutique associée telle la CEA modifiée ou renforcée (CEA lipiodolée ou sousegmentalisée), la perfusion intra-artérielle associée à l'occlusion par ballonnet, la perfusion par lipiodol SMANCS et la perfusion percutanée directe dite de renforcement. En cas de résection, la CEA postopératoire apparaît apporter un plus, alors que la CEA préopératoire ne change pas le pronostic. La CEA et d'autres techniques de radiologie inter-ventionelle peuvent contribuer à l'arsenal thérapeutique grâce à l'aide de la chimiothérapie ciblée. Hemos revisado las bases, técnicas y resultados recientes de las estrategias terapéuticas en el hepatoma. Se han completado las investigaciones experimentales básicas indispensables para justificar la técnica terapéutica y la dosificación. Un total de 1310 casos no resecables fueron sometidos a quimioembolización arterial a través de catéter (TACE) y/o terapias relacionadas; éstas son modificaciones de la TACE, tales como TACE-Lipiodol y como TACE-subsegmentaria, terapias para casos no embolizables como la infusión por arterias ocluidas por balón y la terapia de infusión SMANCS-Lipidol, la inyección percutánea directa que refuerza el efecto de la TACE y otras. En cuanto a los casos resecables, la TACE postoperatoria ha probado ser de beneficio por cuanto aumenta la tasa de sobrevida, en tanto que la tasa preoperatoria no demostró beneficio. La tasa y las terapias de radiología intervencionista relacionadas pueden ser iguales o superiores en el manejo del hepatoma.Keywords
