Abstract
A case is reported of a young man who had been detained within a maximum security hospital following an apparently motiveless knife attack on a young woman. Features of his presentation suggested schizophreniform psychosis with an underlying personality disorder most closely resembling that of sadistic personality disorder. Subsequent investigation and assessment revealed first clcctroencephalographic abnormalities followed by the chromosomal aberration 47 XXY (Klinefelter's Syndrome). The above findings were atypical in comparison with the personality features commonly found in KJinefelter's Syndrome, and raised a subsequent dichotomy with respect to the issue of treatment and further management.
