Effect of glucagon applied topically in the duodenum on the motor activity of the sphincter of Oddi

Abstract
Les auteurs ont étudié chez 9 patients par étude manométrique, l'effet de l'application locale de Glucagon (dose: 1 mg) au niveau de la muqueuse duodénale péripapillaire, sur l'activité motrice du sphincter d'Oddi. Parmi ces patients, 3 présentaient une sténose papillaire (dilatation de la voie biliaire principale avec terminaison effilée et retard d'élimination du produit de contraste iodé). Une lithiase cholédocienne a été constatée dans 1 cas. Ces patients présentaient une pression de repos du sphincter d'Oddi de 39,0±4,20 mmHg et une fréquence des ondes plastiques de 8,5±0,65 par minute, valeurs significativement plus élevées que celles mesurées chez les 5 patients exempts de sténose (pression de repos: 22,80±2,89 mmHg; fréquence: 5,0±0,95 par minute). D'autre part, après administration locale de Glucagon, on ne constate pas de modification significative de l'activité motrice du sphincter d'Oddi pendant la durée de l'étude. Par contre, pression de repos du sphincter a été observée. L'effet hypotonique persistait encore 5 minutes après l'administration de l'hormone. We have studied the action of Glucagon at a dose of 1 mg applied topically in the peripapillar duodenal region, on the motor activity of the sphincter of Oddi in 9 patients with the aid of endoscopic manometry. In three of these cases we found a papillary stenosis (the main bile duct was dilated and showed a tappering end and delayed evacuation of contrast material). In another case we also found choledocholithiasis. Basal sphincter of Oddi pressures in these patients were 39.0±4.20 mmHg, and a phasic wave frequency of 8.5±0.65 per minute, which were significantly higher than those of the 5 patients without stenosis (basal pressure: 22.80±2.89 mmHg; frequency: 5.0±0.95 per minute). On the other hand, there was no significant change in sphincter of Oddi motor activity within the time of the study, following Glucagon administration. On the contrary, in the patients without stenosis we observed a statistically significant decrease in the frequency of the contraction waves as well as that of basal sphincter of Oddi pressure. This effect was maintained up to 5 minutes after the administration of the hormone.Keywords
