Randomised, comparative trial of mecillinam, mecillinam/ampicillin and chloramphenicol in the treatment of enteric fever

Abstract
The efficacy of chloramphenicol, mecillinam alone, and the combination of mecillinam/ampicillin was compared in a randomised study involving 40 patients with enteric fever. All 15 patients given mecillinam/ampicillin responded satisfactorily, and chloramphenicol proved to be effective in 12 out of 13 cases. Only 8 out of the 12 subjects in the mecillinam group were successfully treated. A positive blood culture was obtained before therapy in 24 instances. All the bacterial isolates were sensitive in vitro to mecillinam and mecillinam/ampicillin. Several strains were resistant to chloramphenicol and/or ampicillin. Three patients experienced epigastric discomfort whilst taking pivmecillinam/pivampicillin tablets. Treatment was not withdrawn. No other adverse effects were observed. Further investigation of the combination of mecillinam/ampicillin is warranted.Keywords
