Abstract
Major hazards, especially possible plant failures that may cause many deaths, are receiving increased attention particularly since the explosion at Flixborough. The risk of an accident may be small, but an assessment of its possible occurrence and consequence leads to a general increase in safety. The forthcoming Regulations from the Health and Safety Executive will require notification of the holding of specified materials and may require a hazard assessment. There is a need to extract and correlate lessons not only from accidents, but also from near misses or plant failures which did not lead to an accident. Failure data are being collected and reviewed nationally and internationally.
