Abstract
To the Editor: Recently, two letters in the Journal debated the efficacy of resorting to high doses of antipsychotic medication early in treatment. Baldessarini and his co-workers (N Engl J Med 294:113, 1976) advocate at least two to three months of maintenance on ordinary doses before prescribing higher drug levels. In their reply Anderson and Kuehnle (N Engl J Med 294:670, 1976) opt for a more vigorous "digitalization" approach.1 Although both points of view have merit there is insufficient evidence from well designed, double-blind studies to prove either superior.
