Abstract
A brief review is presented of the role of primary and secondary gain in chronic pain patients. Tertiary gain, someone other than the patient seeking psychological gain from the patient''s illness, is discussed. Case presentations are offered to illustrate the various types of gain and the roles they play in chronic pain problems.
