Cervical myelopathy caused by Whipple's disease

Abstract
Case report. A 62-year-old woman presented in August 1995 with a 7-week history of neck pain and limb paraesthesia. Four weeks after onset, she developed progressive weakness of the arms that later involved the legs. She had urinary retention and constipation. She denied any gastrointestinal complaints, arthralgia, or fever. On examination, her mental state was normal with no cranial nerve dysfunction. She had a spastic quadriparesis with muscle strength of grade 4 in the upper limbs and grade 2 in both lower limbs. She had impaired pinprick to C4 dermatome, but other sensory modalities were preserved. All tendon reflexes were pathologically brisk, and the plantar responses were extensor. She had normal sitting balance but was unable to stand or walk.Keywords
