RENAL GLYCOSURIA: A REVIEW OF THE LITERATURE AND REPORT OF FOUR CASES

Abstract
Pertinent literature from 1900 to 1947 on renal glycosuria was reviewed. The disorder was thought to be not as rare as previously supposed in the light of work done during World War II. Study was made of 4 cases of renal glycosuria in which the rigid diagnostic criteria of Marble were used strictly. In addition, both arterial and venous blood sugars were studied as was serum inorganic P. This demonstrated normal tissue utilization and normal phosphorylation mechanism in these patients. In one instance, a patient developed renal glycosuria subsequent to a series of streptococcic pharyn-geal infections. A tubular lesion was postulated. It was supposed that a vascular lesion of the tubular epithelium could have produced a change in the enzyme system, concerned with the tubular reabsorption rate of carbohydrate. All patients reported had normal glomerular filtration rates. The lesion in renal glycosuria exists in the tubules, lowering the tubular reabsorption rate for glucose. The dangers of treating these patients for diabetes mellitus, particularly in reference to insulin therapy were noted, insulin being more likely to deplete hepatic glycogen stores, producing acetonemia and acetonuria. The importance of liberal carbohydrate intake was stressed. The necessity of studying renal carbohydrate metabolism is noted, as are the already established criteria of Marble in the diagnosing of renal glycosuria.Keywords
