Abstract
The case of a 64-year-old man is presented, who on repeated chest X-ray examinations showed a large wedge-shaped atelectatic area in the right middle lung field. Thor-acotomy revealed a large tumor-like mass involving the lower part of the upper lobe and the entire middle lobe with some involvement of the lower lobe. The microscopic studies showed this lesion to be a lipoid granuloma with multiple areas of necrosis. The chemical examination revealed the presence of saturated paraffin like substance. It is the opinion of the authors, that large lipoid granulomas should be removed surgically, because (1) the areas of necrosis in the granulomas may lead to abscess formation and cavitation, (2) that the presence of paraffin-like substances may lead to the development of lung cancer at some future date. Keywords
