Therapy for Sickle Cell Vaso-occlusive Crises

Abstract
A double-blind clinical trial study, comparing the efficacy of M/6 sodium lactate solution vs N/4 saline solution in the alleviation of painful vaso-occlusive crises, was conducted jointly at Meharry and Vanderbilt hospitals in Nashville, Tenn. On initial entry into the study, the two groups were shown to be similar in demographic and medical characteristics. No statistically significant differences between the two groups in the number of remissions within 48 hours from the onset of therapy and in the duration of crisis from the onset of therapy were observed. Some evidence was found that patients with low initial pH values did better when treated with alkali than with simple hydration alone. The setting in which the patients were cared for may have had an effect on the average duration of crisis. Further study of alkali therapy for treatment of sickle cell crisis may be appropriate.
