Acute hemorrhagic retinopathy occurred in 2 diabetics during the course of pregnancy. The fundi had previously been normal. There was no associated renal toxemia. The retinopathy developed in each case after a spell of vomiting. Vomiting causes a rise in the venous pressure and a fall in the intraocular pressure (due to dehydration). This upset in pressure balance may be sufficient to account for the extensive rupturing of the retinal venules. It is known that the venules are the weak points in the diabetic retinal vascular system.
