Abstract
To the Editor: The use of doxorubicin and vincristine, administered by continuous infusion over four days, in conjunction with dexamethasone to treat multiple myeloma was reported in the Journal in 1984.1 We used this regimen twice in a man with rapidly progressing IgD multiple myeloma, and observed severe cardiac toxicity during both cycles.A 60-year-old white man presented with large osseous plasmacytomas of the ribs, and a diagnosis of IgD multiple myeloma was made. Fifteen years previously he had had a transmural inferior myocardial infarction. On admission he reported stable angina on exertion, controlled with nitrates and metoprolol.
