Temporary resection-arthrodesis of the knee using an intramedullary rod and bone cement

Abstract
A temporary arthrodesis of the knee, using an intramedullary Küntscher rod and bone cement, was carried out in 76 patients after resection of tumours of the distal femur or proximal tibia. In contrast to primdry arthrodesis with bone grafting, this procedure allows the patient to bear weight on a stable limb a few days after operation, reduces the risk of infection and avoids the negative effect of chemotherapy on graft incorporation. In 12 cases (16%) the implant failed mechanically by fracture, or by bending or migration of the rod, at from 13 to 54 months after operation. Deep infection occurred in 11 patients (14%). A second reconstruction was carried out in 14 patients, 10 using Kotz prostheses and 4 arthrodeses with bone grafting. The operation we describe has advantages for patients with high grade tumours which require chemotherapy and resection-arthrodesis of the knee joint. Une arthrodèse temporaire du genou, avec un clou de Kuntscher et du ciment, a été réalisée chez 76 patients réséqués au tiers distal du fémur ou proximal du tibia pour néoplasie osseuse. Cette méthode, comparée à l'arthrodèse immédiate avec greffes osseuses, permet la mise en charge précoce, diminue le risque d'infection et évite les effets défavorables de la chimiothérapie sur la restructuration des greffes. Dans 12 cas (16%) on a eu des échecs (fracture, incurvation ou migration du clou) de 13 à 54 mois après l'opération. Une infection profonde été observée chez 11 patients (14%). Une deuxième reconstruction a été réalisée chez 14 patients (10 prothèses de Kotz et 4 arthrodèses avec greffes osseuses). Cette méthode est avantageuse chez les patients porteurs d'une néoplasie de grade élevé qui nécessite une chimiothérapie et une résection-arthrodèse du genou.Keywords
