Abstract
Presentation of CaseA thirty-nine-year-old physician entered the hospital because of dyspnea and pain in the chest.He had been vigorous and asymptomatic until the onset of fatigue and exertional dyspnea four months previously. Two months before admission he had a respiratory infection accompanied by fever and diffuse aching for two days. Two weeks later he was awakened by severe pleuritic pain in the left side of the chest, with radiation to the left shoulder, and was admitted to another hospital.He had experienced attacks of asthma in childhood.
