Footnotes People present at the California meeting on global health included Richard Feachem (director, Institute for Global Health, San Francisco, and a member of the Commission on Macroeconomics and Health), Julio Frenk (minister of health, Mexico, and one of the people who set up the commission in his time at the WHO), Geeta Rao Gupta (president, International Centre for Research on Women), James Orbinski (past president, Médecins Sans Frontieres), and Nafis Sadik (former executive director, United Nations Population Fund). Since the meeting Richard Feachem has become the head of the Global Fund.4
