Abstract
Cushing's syndrome resulting from primary adrenocortical neoplasms or ectopic ACTH production from nonendocrine tumors is primarily treated surgically. Management of Cushing's disease due to excessive ACTH secretion by the pituitary gland —i.e., secondary bilateral adrenocortical hyperplasia — has been and remains a subject of dispute. The report by Orth and Liddle in this issue of the Journal compares the various modes of treatment in a large number of patients with Cushing's disease and provides an excellent evaluation of the therapeutic alternatives.Adrenalectomy for Cushing's disease has been the mainstay of treatment since 1945.
