Muscle involvement is a frequent manifestation of hypothyroidism (1, 2). Findings include myalgias, stiffness, weakness, elevated muscle enzymes, electromyographic abnormalities, and histologic changes. Serious muscle involvement is distinctly uncommon and rhabdomyolysis has not been recognized previously as a complication of hypothyroidism. We report a case of renal failure produced by massive muscle necrosis and myoglobinuria in a hypothyroid patient. A 60-year-old white woman presented to Milwaukee County General Hospital in October 1976, with complaints of weakness, anorexia, dysuria for 2 weeks, decreased urine output for 1 week, cold intolerance, and numbness in her feet.
