CORRELATION BETWEEN PLASMA CARBAMAZEPINE‐10,11‐EPOXIDE CONCENTRATION AND DRUG SIDE‐EFFECTS IN CHILDREN WITH EPILEPSY

Abstract
SUMMARY: Ninety children with epilepsy were treated with carbamazepine (CBZ) alone or with other anticonvulsant a drugs. Side‐effects were noted in 14 patients. When 25 patients treated with CBZ alone (group 1) were compared with 27 on CBZ and sodium valproate (group 2) and with 38 on CBZ and one or more other anticonvulsants (group 3). the incidence of side‐effects was two in group 1 (of whom one patient had a toxic plasma level of CBZ), but 12 in groups 2 and 3 combined. In all but three of the 14 patients with side‐effects, plasma levels of CRZ were within the ‘therapeutic range’. A significant difference was found between the carbamazepine‐10.11‐epoxide (CBZ‐10,11‐EPOX) levels in plasma and CBZ‐10,11‐EPOX/CBZ ratio in patients with and without side‐effects. For five patients on CBZ and other drugs, changes in treatment resulted in changes in side‐effects, and also in CBZ‐10,11‐EPOX levels. Three of these patients showed an interaction between CBZ and sodium valproate, with a correlation between plasma CBZ‐10.1 I‐EPOX and side‐effects when either drug was introduced or withdrawn, the plasma levels of CBZ itself and of sodium valproate being within the ‘therapeutic range’.RÉSUMÉ: Corrélation entre le taux plasmatique de carbamazepine‐10,11‐époxide et les effets secondaires chez de jeunes épileptiquesQuatre vingt dix enfants épileptiques ont été traités par carbamazépine (CBZ) seule ou associée à d'autres anticomitiaux. Des effets secondaires ont été observés chez 14 sujets. Lors d'une comparaison des 25 sujets traités par la CBZ seule (Groupe 1) avec les 27 sujets traités par CBZ et Valproate de Sodium (Groupe 2) et 38 sujets traités par CBZ associée à un autre anticomitial, l'incidence des effets secondaires a été de deux dans le Groupe 1 (parmi lesquels un sujet avec un taux plasmatique toxique de CBZ) et de 12 dans l'ensemble des Groupes 2 et 3. La taux plasmatiques de CBZ étaient dans la marge thérapeutique normale chez les 14 sujets ayant présenté des effets secondaires, excepté trois. Une différence significative a été trouvée entre les taux plasmatiques de carbamazépine‐ 10,11‐époxide (CBZ‐10,11‐EPOX) et le rapport CBZ‐10.11‐EPOX/CBZ chez les sujets avec ou sans effets secondaires. Chez cinq sujets traités par CBZ et autres anticomitiaux, une modification du traitement a provoqué une modification des effets secondaires et des taux de CBZ‐10,11‐EPOX. Trois de ces sujets ont montré une interaction entre CBZ et Valproate de Sodium avec une corrélation entre le taux plasmatique CBZ‐10,11‐EPOX et les effets secondaires quand l'une ou l'autre drogue était introduite ou retirée, les taux plasmatiques de CBZ et de Valproate Sodium restant eux‐mêmes dans la margé thérapeutique normale.ZUSAMMENFASSUNG: Korrelation von Plasmakonzentrationen von Carbamazepin‐10,11‐Epoxid mit Nebenwirkungen bei Kindern mit EpilepsieNeunzig Kinder mit Epilepsie wurden mit Carbamazepin allein oder in Vergindung mit anderen Antikonvulsiva behandelt. Bei 14 Patienten wurden Nebenwirkungen beobachtet. 25 Kinder wurden nur mit CBZ behandelt (Gruppel), 27 bekamen CBZ und Sodium Valproat (Gruppe 2) und 38 waren auf CBZ und ein oder mehr andere Antikonvulsiva (Gruppe 3) eingestellt. Nebenwirkungen fanden sich in Gruppe 1 bei zwei Kindern (von denen bei einem ein toxischer Plasmaspiegel von CBZ festgestellt wurde), in Gruppe 2 und 3 dagegen bei 12 Kindern. Bei den 14 Patienten mit Nebenwirkungen waren auβer bei drei Kindern die CBZ Plasmaspiegel im therapeutischen Bereich. Ein signifikanter Unterschied fand sich zwischen den Plasmaspiegeln von Carbamazepin‐10,11‐Epoxid und der CBZ‐10,11‐EPOX/CBZ Ratio bei den Patienten mit und ohne Nebenwirkungen. Bei fünf Patienten, die CBZ und andere Medikamente bekamen, führten Ånderungen des Therapieschemas zu Veränderungen der Nebenwirkungen und auch der CBZ‐10,11‐EPOX Spiegel. Drei dieser Patienten zeigten eine Interaktion zwischen CBZ und Sodium Valproat mit einer Korrelation zwischen den Plasmaspiegeln von CBZ‐10.11‐EPOX und den Nebenwirkungen wenn das eine oder andere Medikament dazugegeben oder abgesetzt wurde. wobei die Plasmaspiegel von CBZ und Sodium Valproat im therapeutischen Bereich lagen.RESUMEN: Correlación entre la concentración en sangre de carbamacepina‐10,11‐epóxido y efectos secundarios de los fármacos en niños con epilepsiaNoventa niños con epilepsia fueron tratados con carbamacepina sola o con otros fármacos anticonvulsivantes. Efectos secundarios fueron detectados en 14 pacientes. Cuando 25 pacientes tratados con CBZ sola (grupo 1) fueron comparados con 27 que tomaban valproato sódico junto con CBZ y con 38 que tomaban CBZ y junto con otro o más de uno anticonvulsivante más (grupo 3) se vió que la incidencia de efectos secundarios era de dos en el grupo 1 (con un paciente con niveles tóxicos en plasma de CBZ) y 12 en los grupos 2 y 3 combinados. En todos, excepto tres, de los 14 pacientes con efectos secundarios, los niveles plasmáticos de CBZ estaban dentro de 10s márgenes terapéuticos. Se halló una diferencia significativa entre los niveles de carbamazepina‐10,11‐epóxido (CBZ‐10.11‐EPOX) en plasma y la relación CBZ‐10.11‐EPOX/CBZ en pacientes con y sin efectos secundarios.
