Stages and roentgenological picture*of diabetic osteoarthropathy

Abstract
The course and roentgenographic signs of diabetic osteoarthropathy are described on the basis of literary data and of 23 cases of the author. Three stages are distinguished: in stage I dislocation, circumscribed porosis and cortical defect are observed. The characteristic sign of stage II is osteolysis, accompanied by fragmentation, fracture, periosteal reaction. In stage III the healing takes place according to five radiologically distinguishable patterns. The good healing tendency is typical. Regarding the differential diagnosis of the disease, the possibility of inflammatory and tumourous changes and neurothropic osteoarthropathies of other origin must be considered. Der Ablauf und die Röntgenerscheinungen der Osteoarthropathia diabetica werden aufgrund der Fachliteratur und der Untersuchungen von 23 Patienten des Autors besprochen. Es werden drei Stadien unterschieden. Im ersten Stadium kann eine Luxation, eine lokalisierte Osteoporose und ein kortikaler Defekt beobachtet werden. Die charakteristische Erscheinung des zweiten Stadiums ist die Osteolyse mit Begleiterscheinungen wie Fragmentation, Frakturen und periosteale Reaktion. Das dritte Stadium ist die Heilung, die in fünf verschiedenen radiologischen Formen verläuft. Die Neigung zu einer guten Heilung ist charakteristisch. In der Differentialdiagnose kommen Veränderungen durch Entzündung oder Tumor bzw. neurotrophische Osteoarthropathie mit verschiedenem Ursprung in Frage.
