Replacement of femoral head endoprosthesis with total hip prosthesis A report of five cases with problems of management

Abstract
Five cases who had endoprosthesis implanted for medial fracture of the neck of the femur are reported. All of them developed late complications that required replacement of endoprosthesis with total hip prosthesis. Complications included wearing of both joint cartilage and subchondral bone of the acetabulum, osteoporosis of the proximal end of the femur and fracture of the femur at the level of the endoprosthesis. Several problems of surgical management ensued. This experience strengthens the authors' opinion that other forms of treatment—reduction in traction and nailing or total hip prosthesis—should be tried first also in particular cases of chronologically aged, but physically healthy patients with medial fractures of the neck of the femur. Es werden 5 Fälle von medialen Schenkelhalsfrakturen beschrieben, die mit Femurkopf-Endoprothesen versorgt wurden. Alle hatten später Komplikationen, die die Auswechslung der Endoprothese mit totalem Hüftgelenkersatz erforderten. Die Komplikationen waren: Verschluß des Gelenkknorpels und der subchondralen Knochenfläche des Acetabulums, Osteoporose der angrenzenden Femurbezirke und schließlich Frakturen auf Höhe der Endoprothese. Jede Komplikation erforderte eine andere chirurgische Behandlung. Aufgrund dieser Erfahrungen verstärkt sich die Meinung der Autoren, daß, in besonderen Fällen, selbst bei Patienten in höherem Alter, aber in verhältnismäßig gutem Allgemeinzustand, bei der medialen Schenkelhalsfraktur Extensionsbehandlung, primäre Nagelung oder ein totaler Huftgelenkersatz versucht werden sollten.
