Diabetic Glomerulosclerosis in Renal Papillary Necrosis—An Autopsy Study

Abstract
It has been suggested that diabetic glomerulosclerosis is uncommon in patients with renal papillary necrosis, and that the finding of diabetic glomerulosclerosis in a renal biopsy might be useful in excluding papillary necrosis as a cause of acute renal failure in a diabetic. Forty-six cases of renal papillary necrosis diagnosed at autopsy were reviewed. Seventeen of these cases were known to be diabetic. Thirteen of the 17 had concomitant diabetic glomerulosclerosis, a total incidence of 76%. A selective review of the literature showed an overall incidence of diabetic glomerulosclerosis in diabetics with renal papillary necrosis of 41%. The presence or absence of diabetic glomerulosclerosis cannot serve as a differential point in ruling out the presence of papillary necrosis in a diabetic with acute renal failure.
