ARTERIOVENOUS-MALFORMATIONS IN DEEP CEREBRAL AREA - TOPOGRAPHIC CLASSIFICATION - THERAPEUTIC POSSIBILITIES AND RESULTS ABOUT 46 CASES

Abstract
The clinical dispositions of 46 arteriovenous malformations [AVM] in deep cerebral area were studied. They were detected in 41 cases due to hemorrhagic accidents. After radiological analysis anatomoradiological classification was made. AVM of the nucleus centralis were classed together with the localizations of the nucleus caudate''s head, the insula and lentiformis nucleus and the capsulo thalamic area, the ventriculo-cisternalis AVM, the corpus callosum AVM and complicated forms. This classification allowed analysis of different operative techniques permitting complete extirpation in 26 cases of 39 operated patients. Results were excellent in 19 cases, good in 8 cases and mediocre in 4 cases; 8 patients died postoperatively. The most important factors for the final outcome were consciousness disturbances before operation and the development of malformations in several anatomical areas. If embolism techniques are not effective, the operation is indicated in spite of the technical difficulties.
