RENAL URATE EXCRETION IN 5 CASES OF HYPOURICEMIA WITH AN ISOLATED RENAL DEFECT OF URATE TRANSPORT

Abstract
Renal urate excretion was studied in 2 familial, 1 suspected familial and 2 isolated cases of hypouricemia due to a renal defect. All had very low plasma urate concentrations. In 4 cases, the urate clearances were approximately the same as the creatinine clearances and in 1 case reduced to about 1/3. In all cases the urate clearances were minimally diminished by both pyrazinamide and probenecid. This renal response to the drugs was probably due to an isolated tubular defect in the reabsorptive transport mechanism of urate. Following i.v. administration of uric acid, 1 patient excreted uric acid in the urine more rapidly than a normal subject. In this patient, uric acid secretion from renal tubules was clearly demonstrated during infusion of uric acid. In de novo synthesis of purines, no definite abnormalities were found by incorporation of 15N-glycine to uric acid.This publication has 2 references indexed in Scilit:
