Role of Phosphate in the Genesis of Secondary Hyperparathyroidism of Renal Failure

Abstract
The data pertinent to the role of phosphate retention in the genesis of secondary hyperparathyroidism in patients with incipient renal failure are reviewed and the possible mechanisms involved are discussed. The factors underlying the hypocalcemia and secondary hyperparathyroidism in patients with early renal failure apparently are multiple. The role of phosphate retention in this process is probably not that of a direct effect on the serum concentration, but an indirect one mediated through derangements in the production of 1,25 dihydroxy vitamin D3.
