Abstract
Renal response to acute ingestion of ammonium chloride and to fluid restriction was investigated in 12 patients with systemic lupus erythematosus and normal glomerular filtration rate. No overt evidence of acid-base imbalance was found in any of the patients, but significant impairment of renal acid excretion was noted following the acid load, indicating latent renal tubular acidosis. Impairment of renal concentrating ability was also noted. A possible relationship of the observed renal tubular dysfunction to hypergammaglobulinemia was discussed.
