Erfahrungen mit der Leitlinie »Brennen beim Wasserlassen« - Eine Anwendungsbeobachtung

Abstract
Die Intention unserer Verlaufsbeobachtung war, die Anwendungshäufigkeit und Effektivität der DEGAM-Leitlinie »Brennen beim Wasserlassen« zu prüfen. Vom Frühjahr 2001 bis Frühjahr 2002 wurden 212 Harnwegsinfekte in neun Arztpraxen in Bremen und Umgebung behandelt. Der Verordnungsanteil von Trimethoprim (TMP) lag mit 67,9 % relativ hoch. Es konnte gezeigt werden, dass Harnwegsinfekte entsprechend der Leitlinie in einem hohen Prozentsatz erfolgreich behandelt werden konnten. Die Antibiogramme ergaben für TMP eine fast genauso hohe Sensibilität wie für Cotrimoxazol (81,7 % versus 82,5 %). Eine Sensibilität gegenüber Ciprofloxacin fanden wir in 90 %. Die Umsetzung der Leitlinie erwies sich in einigen Praxen als schwierig, insgesamt führte jedoch unsere Untersuchung zu positiven Erfahrungen mit der Leitlinie und damit zu einer Steigerung der TMP-Verordnungshäufigkeit. The intention of our course observation was to investigate the application and effectiveness of the German Society of General Practice and Family Medicine (DEGAM) guideline »burning sensation when passing water«. 212 cases of urinary tract infection were treated in nine practices in Bremen and its environs from early 2001 to early 2002. The prescription share of trimethoprime (TMP) at 67,9 % was relatively high. The results indicate that the urinary tract infection was successfully treatable at a high rate in line with the guidelines. The antibiogrammes indicated just as much sensitiveness to TMP as to cotrimoxazol (81,7 % versus 82,5 %). Sensitiveness to ciprofloxacin was at 90 %. The application of the guidelines in certain practices proved difficult. Generally, however, our research indicated some positive experience with the guidelines and thus the increase in the prescription of TMP.
