XIII Auditory Manifestations of Cochlear and Retrocochlear Lesions in Humans

Abstract
Auditory manifestations of lesions involving the end-organ of hearing of the auditory pathways are reported. The precise diagnosis of deafening diseases cannot be made without a complete otoneurological study of the patient, including adequate vestibular testing, roentgenograms, angiograms and cerebrospinal fluid studies, but from the foregoing the following important points emerge: lesions of the sensory cells in the end-organ of hearing have their maximum effect upon the pure-tone threshold; lesions of the 1st-order neuron and VIII nerve have their maximum effect upon speech discrimination and auditory adaptation; and, supranuclear lesions have their maximum effect upon auditory adaptation and speech discrimination. The latter can be detected only by special discrimination tests.
