Ocular Findings in Multiple Myeloma

Abstract
The clinical and histologic findings in the eyes of two patients with multiple myeloma are described. In case 1, the patient had angle closure glaucoma that was possibly related to (1) relaxation of the zonular fibers and anterior displacement of the lens, (2) anterior displacement of the iris base by corona ciliaris cysts, (3) recurring iridocyclitis, or a combination of all three. In case 1, it was noted that the cysts were clear in vivo and that they became opaque after enucleation and fixation in formaldehyde solution. In case 2, due to the debilitated state of the patient, unilateral metastatic bacterial (pneumococcal) endophthalmitis and meningitis were superimposed on the commonly observed ocular manifestations of multiple myeloma. The associated ipsilateral disc edema appeared to be due to inflammatory causes rather than to the consequences of an intracranial or intraorbital plasmacytoma.
