Abstract
Presentation of CaseA 68-year-old truck driver was admitted to the hospital because of dyspnea.He had been well and assiduously avoided physicians all his life until two years before admission, when exertional dyspnea, orthopnea and ankle edema developed. He gave up his customary two packages of cigarettes daily and consented to see a physician, who found enlargement of the liver and prescribed a diuretic agent, with improvement. Ten months before admission the dyspnea increased in severity so that he was unable to walk a block; he consulted another physician, who advised a thorough evaluation, but the patient declined.
