Successful transplants across T warm‐positive crossmatches due to IgM antibodies

Abstract
Thirteen kidney transplants and 1 heart transplant were success fully performed across T warm‐positive crossmatches due to IgM antibodies. Such IgM antibodies were clearly distinguished from IgG antibodies by using dithiothreitol (DTT). Among 149 sensitized recipients, 39 (26%) appeared to have such DTT‐sensitive antibodies, that is, their PRA fell by more than 20% after the DTT treatment. DTT‐sensitive antibodies were found more in non‐transplanted patients, in less transfused recipients, in male patients, in aged patients, in SLE patients, and in patients awaiting organ transplantation for less than 1 year. These patients should be crossmatched using DTT‐treated sera and could be encouraged to undergo transplantation if crossmatch is negative after the treatment.
