Clinical and radiographic evaluation of modular knee replacement

Abstract
Ninety-five modular total knee replacements, 54 of them unicompartmental, have been reviewed. The average follow-up was three years. The major indication for operation was pain. A precise preoperative radiographic evaluation and a technique for a positioning the components are described. This had led to a significant improvement in the clinical and radiographic results in 67 out of 95 knees which were operated on. The modifications concern the orientation of the tibial cut, the placing of the femoral components and the correction of axial deviation. The main causes of the 13% of failures in our series were either errors in positioning the components or a preoperative diagnosis of rheumatoid arthritis. Unicompartmental replacement gave similar results in both valgus and varus knees, and the morbidity was lower than in the bicompartmental replacements. The best results were obtained in osteoarthristic knees in elderly patients and in posttraumatic osteoarthritis. Les auteurs ont analysé 95 arthroplasties partielles du genou de type modulaire dont 54 remplacements uni-compartimentaires, avec un recul moyen de trois ans. L'indication majeure a été la suppression de la douleur. L'étude des résultats cliniques et du positionnement radiologique des prothèses, a objectivé une amélioration notable de ces derniers depuis l'utilisation d'une nouvelle procédure instrumentale et d'un calcul radiologique pré-opératoire concernant 67 des 95 genoux opérés. Les modifications essentielles ont porté sur l'orientation de la coupe tibiale, le centrage condylien et la correction de la déviation axiale. Les défauts de positionnement prothétique et l'arthrite rhumatoïde ont représenté les principales causes des 13% de mauvais résultats observés dans cette série. Les remplacements uni-compartimentaires ont donné des résultats équivalents dans les déviations en varus et en valgus et leur morbidité s'est avérée moindre que celle des remplacements bi-compartimentaires. La gonarthrose avec perte de substance osseuse condylienne ou tibiale des sujets âgés et les séquelles post-traumatiques représentent les indications électives de ce type de prothèse.Keywords
