INSULIN RESISTANT DIABETES-MELLITUS WITHOUT PRESENCE OF INSULIN-ANTIBODIES - CASE-REPORT

Abstract
A case of a 26 yr old woman suffering from an insulin resistant diabetes mellitus for 14 yr is described. Acanthosis nigricans was diagnosed in the patient''s 2nd yr and the Stein-Leventhal syndrome at the age of 15. Diabetes could not be properly controlled either with daily doses of insulin as high as 1140 units or with peroral tolbutamide. Fasting serum immunoreactive insulin concentrations were elevated; the secretory response to the stimulation by glucose or tolbutamide was substantial and prolonged. The hypoglycemic response to the i.v. application of commerical insulin was insignificant. Serum growth hormone levels were normal.
