Acquired Cone Dysfunction

Abstract
This paper describes a case of a woman who developed an apparently stationary form of generalized cone dysfunction without observable fundus changes following therapy for systemic infection complicating cesarean section. The patient can therefore be clearly distinguished from other reported cases of idiopathic cone dysfunction which are usually progressive and associated with macular degeneration or generalized retinopathy. Psychophysical and electroretinographic (ERG) evaluation revealed a widespread disorder of the photopic mechanism. There was a decrease in Snellen acuity, disturbance in hue discrimination, constriction of color fields, and absence of ERG flicker responses. The rod mechanism, on the other hand, appeared to be uninvolved. The possible implication of drugs or toxins in producing a selective disorder of cone function is discussed.
