Abstract
The recording of adverse drug reaction as diagnoses for the Hospital Activity Analysis (H.A.A.) over a four year period at the Hereford Hospitals is described. H.A.A. data is recorded for all in‐patients in England and Wales, and thus represents a large data base. It is suggested that this information could be used to encourage the reporting of adverse reactions to the Committee on Safety of Medicines by alerting the pharmacist to the occurrence of reactions.This publication has 5 references indexed in Scilit:
